Once you register for an account at ECOMP - U.S. Department of Labor (dol.gov), you can submit an OSHA 301 first and then a CA-1 for COVID.
Date of Injury – Enter the last date that you worked and were exposed to other people in the work setting, prior to the onset of COVID-19 symptoms or a positive COVID-19 test result. (Other people may include patients, member’s of the public or co-workers.)

You must file your CA-1 for COVID within 30 days of the date of injury to be eligible for Continuation of Pay (COP) leave.
You can create an account at https://www.myhealth.va.gov and obtain your test result from there. Once you have your test result, please attach a copy to your CA-1 for COVID.
